Nursing Home Transition

Nursing Home Transition is a Pennsylvania statewide initiative designed to provide early identification and education to newly admitted residents (and their families) of nursing homes facilities.  Within 30 days of initial admission, YCAAA staff will provide Long Term Living counseling regarding available options on alternative sources of long term care services to those people who wish to transition from traditional nursing home residency.
