Long-Term Care

Long-term Care Programs is the umbrella for services designed to assist individuals who clinically require nursing facility care but would like to remain living in the community.  The Nursing Home Transition and PDA Waiver programs are described below.  A third service under this category is Pre-Admission Assessment to determine the least restrictive placement recommendation.
