SeniorLIFE is a program designed specifically to sustain independence among seniors by coordinating and/or providing a comprehensive range of support services all through a single network.  Dedicated, qualified health professionals will get to know you personally.  A wide array of services are provided that are customized to accomodate the special needs of each individual. 

Eligibility Criteria

• 60 years of age
• Reside in the area served by SeniorLIFE York
• Determined to be medically eligible by the York County Area Agency on Aging
• Determined by SeniorLIFE York to be able to be safely served in the community
• Eligible for Medicaid or be able to pay a flat monthly fee

SeniorLIFE's services include:

• Physician services
• Nursing
• Therapies
• Medication monitoring
• Recreational therapy
• Meals and nutritional counseling
• Social services and counseling
• Home care services
• Transportation
